Modified Marshall Scoring System for Organ Dysfunction Banks et al. 2013 Modified Marshall Scoring System for Organ Dysfunction

* Serum creatinine units:
Qscore
* Renal (serum creatinine μmol/lmg/dl )

A score for patients with pre-existing chronic renal failure depends on the extent of further deterioration of baseline renal function. No formal correction exists for a baseline serum creatinine ≥134 μmol/l or ≥1.4 mg/dl.

* Respiratory (PaO2/FIO2)

For non-ventilated patients, the FiO2 can be estimated from below:
Supplemental oxygen (l/min) → FiO2 (%)
Room air → 21
2 → 25
4 → 30
6-8 → 40
9-10 → 50

The determinant of the severity of acute pancreatitis during the early phase is primarily the presence and duration of organ failure.
- Presence: Organ failure is defined as a score of 2 or more for one of the three organ systems (respiratory, cardiovascular and renal)
- Duration: This is described as transient organ failure if the organ failure resolves within 48h (moderately severe acute pancreatitis) or as persistent organ failure if organ failure persists for more than 48h (severe acute pancreatitis)
If organ failure affects more than one organ system, it is termed multiple organ failure (MOF).
